A concept paper, explaining the model that underpinned the London Housing Foundation's programme to bring about systemic change in the use of outcome monitoring by agencies tackling homelessness. The paper identifies the need to work on a number of levels simultaneously: working with individuals, with organisations, on systems and procedures and at the policy level.
